- To provide psychological and emotional support for you and your relatives.
- To provide education and training for staff.
- To act as a link with the Midlands Centre for Spinal Injuries for your referring hospital.
- To plan your admission with your team for your rehabilitation.
The Outreach Team plays a vital role in ensuring a smooth transition and providing comprehensive support throughout your care journey. After receiving your referral, one of our multi-disciplinary team members will carry out an initial assessment, typically within 5-10 days. We will liaise with your care team and provide specialist advice to help plan your care effectively.
Additionally, we offer psychological and emotional support for both you and your relatives, as well as education and training for staff involved in your care. The Outreach Team acts as a key link with the Midlands Centre for Spinal Injuries and your referring hospital, ensuring coordinated communication throughout your care. We collaborate closely with your team to plan your admission and rehabilitation, making certain that the process is seamless and supportive at every stage.
